The Bridge Project gives cash support to pregnant moms. Selected mothers receive consistent cash, no strings attached, for up to three years. This application is for pregnant people in Milwaukee, Wisconsin. To complete this application, you will need to provide your name, contact information, birthday, due date, address, and income. This information will be used to determine your eligibility for the program.
You will also need to upload a document proving your pregnancy. Your proof of pregnancy must include your full name, your provider's name, the date of your visit (within the past 3 months), and your due date or how far along you are. A MyChart screenshot, after-visit summary, WIC referral form, or urine or blood test result all work.
Any information you submit will remain confidential. If your application is selected to move forward, we will contact you with next steps.
If you are filling this application out on behalf of someone else, please enter information for the applicant, not yourself.
Required fields are marked with an asterisk (*).
This helps us follow up with you and confirm next steps.
If we can't reach you, this is who we'll contact instead. It can be another number or email of your own, or someone you trust — like a case worker, family member, or friend — who the Bridge Project can share information with.
Please share the address where you currently live. Your address is used to determine your eligibility for the program.
Please upload a document verifying your pregnancy, such as a MyChart screenshot, after visit summary, WIC referral form, or urine or blood test results. The document you upload must include your full name, your provider's name, the date of your visit (within the past 3 months), and your due date or how far along you are.
Household income includes income for you and your spouse, if applicable. It does not include income from other people you live with, such as parents, grandparents, or other relatives.
If you are not sure of the exact number, or cannot access it right now, enter your best estimate. We will verify your income if you move to the next stage.
This question helps us support you if you are selected to move forward. Your answer does not affect whether you are selected.
These questions are optional. Your answers won't affect your application.
