Welcome to the Bridge Project application

The Bridge Project gives cash support to pregnant moms. Selected mothers receive consistent cash stipends, no strings attached, for up to three years. This application is for pregnant people in Chattanooga, Tennessee. To complete this application, you will need to provide your name, contact information, birthday, due date, address, and income. This information will be used to determine your eligibility for the program.

You will also need to upload a document proving your pregnancy. Your proof of pregnancy must include your full name, your provider's name, the date of your visit (within the past 3 months), and your due date or how far along you are. A MyChart screenshot, after-visit summary, WIC referral form, or urine or blood test result all work.

To be accepted, you must meet the following requirements:

• Be 18 years of age or older
• Be 20 weeks pregnant or less
• Live in Hamilton County, Tennessee
• Meet the income requirements for TennCare (Tennessee Medicaid). You do not need to be currently enrolled.

Any information you submit will remain confidential. If your application is selected to move forward, we will contact you with next steps.

Please upload a document confirming your pregnancy. Accepted documents include:

• A screenshot of your MyChart after-visit summary
• An after-visit summary from your healthcare provider (MD/DO/NP/PA)
• A WIC referral form
• Urine or blood test results issued by a healthcare system or laboratory, such as Labcorp or Quest

Your document must include:

• Your full name
• Your provider’s name
• The date of your visit, which must be within the past three months
• Your due date or how many weeks pregnant you are
• Your provider’s signature, as applicable

If your MyChart screenshot or after-visit summary does not include a handwritten or electronic signature, you can still submit it. We’ll review it individually and may ask for additional documentation.
